This RN will be primarily responsible for utilizes nursing assessment skills effectively and consistently demonstrates competent clinical skills. Coordinates and completes patient assessment and re-assessment to positively impact on quality outcomes.
* Assesses barriers to learning and functional status of patient. Coordinates, evaluates, and completes patient/family teaching.

Principal Accountabilities:
- Promotes better patient outcomes by reviewing open gaps of care and coordinating closure of care gaps. Interacts directly with patients and team members, documents in Epic as appropriate
- OB chart prep / pending orders for clinicians
- Responsible for in basket management. Assists with pharmacy contact, medication ordering and renewal. Reviews and appropriately disseminates labs based on acuity. Reviews and appropriately responds to patient messages, in collaboration with clinician(s).
- RN should triage all calls as needed. Responsible for immediate response to ALL emergent calls. (In-basket messages: patient calls, patient advice request).
- Daily huddle with clinical staff to review tasks for the day, any coverage that may be needed: set the day up for success.
- Recognizes emergency situations and acts appropriately. Clarifies physician orders as indicated. Maintains a safe environment for team members and patients/visitors: physical, psychological, and environmental. Utilizes the HRO safety principles.
- Care Navigator: monitoring of OB list, ensuring it is updated including all low risk/high risk. Ensure follow through on labs/US/ consults. Follow up with patient during pregnancy regarding education - providing too much up front can be overwhelming, divide education throughout appropriate intervals of pregnancy.
- Demonstrates knowledge of medication and vaccine administration according to policy, carries out physician orders in an accurate and timely manner. Monitors effects of medications and reports adverse effects in a timely manner.
- Complete NSTs in office
- Post-partum follow-up - ensuring patients are seen for post-partum visit and have access to any resources they may need.
- Demonstrates knowledge of policies and procedures, including but not limited to knowledge and skill of emergency equipment and procedures. Identifies and reports all risk management issues in a timely manner.
- Contact patients regarding results per clinician instructions (GYN and OB), ensure documentation is completed in the patient chart.
- Assist with in office procedures as needed and provide CMA coverage as needed.
- Participates in and/or organizes regular practice huddles, communicates effectively with team members and physicians; actively listens and seeks feedback. Collaborates with team members to solve problems and resolve conflicts. Functions as a resource/mentor and provides support to team members to resolve patient care and team concerns/issues.
- Utilizes standard precautions including, appropriate isolation techniques. Consistently follows the National Patient Safety Goals.
- Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidence based care within a healing culture for the patient population served and their families.
- Demonstrates knowledge of documentation guidelines. Documents patient's response to the ordered treatment protocol. Reports changes in a patient condition to the physician immediately. Consistently collects and completes all required documentation. Verifies accuracy of order entry and transcription. Maintains competency in use of all needed information systems. Able to communicate effectively verbally and electronically.
- Pursues and participates in formal and independent learning activities to enhance knowledge, clinical competency, and professional development.
- Performs other duties per operational needs of the assigned clinical practice area and within the scope of RN practice.
- Supports the overall strategic goals of the clinical practice care area, AMG, the AHS hospital sites, Atlantic Visiting Nurse, and the system.

Required Education:
- High School Diploma/GED; graduate of an accredited School of Nursing required; Bachelor's degree in Nursing
Required License, Certification
- RN license, valid in the State of N.J.
- Certification- American Heart Association Basic Life Support (BLS) credential required
