Duties

• These positions report to the Social Work Service of the Bay Pines VA Healthcare System
• The duties and responsibilities are carried throughout the Healthcare System including all clinical and other patient care areas involved with the services
• Senior Social Worker-Healthcare for Homeless Veterans (HCHV) The Health Care for Homeless Veterans (HCHV) Social Worker supports the VA Homeless Program by providing continuity of care for homeless Veterans between VHA and community-based programs - as the Veteran transitions from homelessness into programs such as HCHV Contract Residential Housing (CRS) - Residential Rehabilitation and Treatment Programs (RRTPs) - and Grant Per Diem Programs (GPD) as well as directly to permanent housing through the Housing and Urban Development-Veterans Affairs Supportive Housing (HUD-VASH) Program - the Supportive Services for Veteran Families (SSVF) - or other forms of housing in the community
• Senior Social Worker-Community Care VA provides care to Veterans through community providers when VA cannot provide the care needed
• Community care is based on specific eligibility requirements - availability of VA care - and the needs and circumstances of individual Veterans
• Senior Social Worker duties include - but are not limited to: Incumbent performs specialized treatment and care coordination for individuals with complex medical or mental illness
• Incumbent incorporates complex multiple causation in differential diagnosis and treatment of veteran patients - including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice in the Emergency Department - Inpatient Wards - and Outpatient specialty clinical settings
• The Senior Social Worker has technical - administrative and clinical responsibility in various clinical settings to include: crisis intervention - delivery of psychosocial services - managing complex discharge patients - conducts thorough comprehensive suicide assessments and safety plans as indicated - evaluates for needed follow-up services - makes community referrals - and provides psychosocial consultation to interdisciplinary team
• This worker is responsible for the implementation of written treatment program policies and procedures developed by Social Work Service that are based on coherent empirical and theoretical rationale and are consistent with patient rights and the maintenance of comprehensive written patient progress notes - outcomes and discharge planning services in collaboration with the unit treatment team
• Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas - utilizing outcome evaluations to improve treatment services
• Incumbent is able to demonstrate the ability to design changes based on empirical findings
