Location: Twin Falls, Idaho | St. Luke's Clinic - Post-Acute Medical Group

Join our SNF team in Twin Falls, where you'll provide transitional care in Skilled Nursing Facility (SNF) settings, helping patients and families navigate recovery after hospitalization.

Key Responsibilities:

• Provide care through clinical judgment, diagnostics, and treatment planning
• Manage acute and chronic conditions in the SNF setting
• Maintain clear, complete documentation to support continuity and quality of care
• Participate in team meetings, peer reviews, and continuous learning initiatives

Qualifications:

• Graduate of an accredited NP or PA program; eligible for Idaho licensure
• DEA and prescriptive authority (or ability to obtain)
• Strong interest in internal medicine, geriatrics, and transitional care
• Excellent communication and collaboration skills
