Job Summary
Under minimal supervision, responsible for the management and evaluation monitoring of the accuracy and integrity of patient health information and documentation in the electronic legal medical record maximizing quality of care utilized in the storage, retrieval, data abstraction, reporting, payer roster management, and interoperability accessibility ensuring confidentiality of protected/electronic health information. Requires research, reconciliation, and resolution of patient identity issues in the electronic health record and Master Patient Index (MPI). Correction of duplicate medical record numbers, erroneous patient overlays, provider documentation correction, patient amendments, managing regulatory interoperability data and external health exchange information reconciliation/sharing via interface error resolution. Facilitates enterprise education for erroneous patient arrival/documentation correction collaborating with Patient Safety Quality Department and Clinical Educators. Collaborates routinely with Enterprise Compliance Service Department to research and verify suspected Identity Theft. Serves as the SME and only team with security to perform patient medical record merging and contact moves of documentation upon thorough analysis and verification.
Essential Functions:
Responsible for maintaining patient identity integrity using analytical and investigational skills to coordinate and execute action needed to remediate identity discrepancies. (20%)
Responsible for monitoring work queues, requests, and resolves enterprise HELP Desk patient identity tickets requiring maintenance in the EHR. (20%)
Facilitates the chart correction process in the legal medical record for patient safety and quality. Collaborates with end users to ensure appropriate Electronic Health Record (EHR) documentation errors are corrected in a timely manner per regulatory requirements. Serves as the patient identity SME for Patient Access and clinical departments. (20%)
Improves patient safety and quality by researching and resolving duplicate patient medical record numbers, merging of patient accounts and/or separating documentation in the legal medical record when a patient overlay has been erroneously performed. (20%)
Conducts validation process and data audits ensuring data accuracy is maintained throughout the EHR documentation capture process. Assists in establishing procedures and best practices for documentation integrity and reconciliation. (10%)
Performs investigative research to assist with suspectable identity fraud and resolution for the Identity Theft Committee and performs moving of EHR documentation and compiling of correspondence to the victim as required. (10%)
Education:
Associate's Degree Health Information Technology or related healthcare field of study Required
Work Experience:
1-3 years of related experience Required
Knowledge, Skills and Abilities:
Ability to analyze data and make critical thinking decisions based on information gathered.
Ability to understand clinical, demographic, payer, and data throughput information to reason and solve problems.
Strong written and verbal communication skills required; including collaboration with providers as necessary.
Frequent interaction with providers and hospital/clinical staff will be expected for follow up and training opportunities are identified.
Proficient in Microsoft Office Suite.
Knowledge of Electronic Health Record Applications and Health Information Systems.
Database skills.
Epic/OnBase knowledge preferred.
