Overview

• Collaborate with healthcare teams to deliver safe, effective patient care and discharge planning.
• Assess patients' and families' psychosocial needs, including prior functioning, support systems, and reactions to illness.
• Counsel patients and families regarding emotional, social, and financial impacts of illness or disability.
• Access and mobilize community and family resources to meet patient needs.
• Conduct mental health assessments upon physician referral and provide therapeutic interventions.
• Support complex discharge planning including guardianship, foster care, adoption, and mental health placements.
• Serve as the primary point person for cases involving child abuse/neglect, family violence, institutional abuse, and domestic violence.
• Liaise with protective services, law enforcement, schools, probation, and other agencies.
• Utilize crisis de-escalation, interviewing, and clinical intervention techniques to support dangerous or high-stress situations.
• Lead clinical interventions during emergencies to ensure safety and stability.
• Develop and maintain strong relationships with community partners and healthcare teams.

• Education: Master's Degree in Social Work required.
• Licensure: Permanent LSW licensure required; LCSW licensure preferred.
• Certifications: De-escalation certification required within 6 months of hire.
• Experience: 3-5 years preferred in mental health, child and family services, adult protective services, Veteran's Administration, or substance abuse.
• Additional Skills & Knowledge:
  • Knowledge of the disease impact and human behavior for assessments, crisis intervention, and counseling.
  • Understanding of legal mandates related to protective services.
  • Principles of growth and development across the lifespan.
  • Strong crisis de-escalation and intervention skills.
  • Ability to develop relationships with community agencies, law enforcement, and healthcare providers.
  • Proficiency in Microsoft Excel and Word.
