The role you'll contribute: The Post-Acute Care (PAC) Liaison will function as a patient advocate as it relates to coordination of post-acute care services with the members of the AHS Preferred Provider Network (PPN). This person will assist in identifying patients with post-acute care needs, will collaborate with multiple disciplines to coordinate post-acute services, and maintain the relationships with post-acute providers. This person will manage the daily activity of the PPN, including data collection and analysis, and work closely with the Transitional Care Coordinator (TCC) on readmission management and quality monitoring.

- Adheres to mandated federal, state, and local regulatory and statutory requirements as well as agency or facility compliance policies and procedures for referral processing.
- Follows up with PPN-affiliated patients and the respective post-acute care provider when patients are re-hospitalized during their post-acute care episode for quality assurance.
- Identifies patients with post-acute care needs and collaborates with Care Management, Care Navigation, and the Transitional Care Coordinator (TCC) to coordinate these services.
- Acts as a liaison between the hospital and post-acute agencies affiliated with the local Preferred Provider Network (PPN).
- Coordinates with Care Management and other departments to ensure seamless patient transitions.
- Monitors patient outcomes and satisfaction to improve post-acute care services.
- Develops and implements strategies to enhance the efficiency of post-acute care processes.
- Provides training and support to staff on post-acute care protocols and procedures.
- Other duties as assigned.
- Maintains a high level of competency, growth, and development.
- Promotes the growth of post-acute positive public relations and excellent customer service skills.
- Collects, analyzes, and presents PPN data to hospital and PPN agencies on a regular basis.
- Educates patients and families on the Preferred Provider Network and advocates on behalf of the patient.

- Technical/Vocational School Required
- 3 Work Experience Required
- Licensed Practical Nurse (LPN) Required
- LPN - Licensed Practical Nurse - State Licensure / Current registration with Florida State Board of Nursing Required
