Overview

• Manage and maintain a registry of all patients interacting with the behavioral health team.
• Track and document time spent on patient care activities.
• Support behavioral health interventions initiated by care teams, including screening and response measurement.
• Provide information on internal and external resources to patients and staff.
• Coordinate referrals for sites not physically staffed by the behavioral health team.
• Collaborate with the integrated behavioral health team to achieve clinical and cost outcomes.
• Identify and triage crisis needs promptly.

• High School Diploma or GED required.
• Associate's degree in a medical or behavioral health field or certification as a Medical Assistant required.
• Certifications:
• BLS (Basic Life Support) certification required.
• Skills & Experience:
• Strong organizational skills and attention to detail.
• Ability to problem-solve and work independently.
• Proficiency with EMR programs.
• Experience in medical office or behavioral health settings is a plus.
• Microsoft Office experience is a plus.
