Job Summary:
The Network Senior Social Worker will provide medical or mental health related supportive services to patients at Westchester Medical Center Health Network. The Network Senior Social Worker role is primarily based in the outpatient setting, including the Center for Women's Health Equity and APS Practice sites with some minor inpatient service to ensure continuity of care across clinical settings. The candidate will be required to perform complex care coordination within the WMC Health Network but will be based out of the main campus in Valhalla, NY. The social worker will work for the Center for Women's Health Equity and the department of OB/GYN.
Responsibilities:
- Appraises the psycho-social assessment of the patient/family system to determine strengths and weaknesses, and for specialized resources available to them in order to develop and implement a plan of action suitable to the patient.
- Interviews patients and family members to determine their problems and needs as they perceive them.
- Provides supplements to the medical history of a patient when needed, or when such is disclosed by investigation and is relevant to the case plan.
- Coordinates hospital care as well as the discharge planning function, including referrals to home health care service providers, residential health care facilities, psychiatric and medical rehabilitation hospitals and programs, and other community supports.
- May represent the hospital at community meetings and act as a liaison and coordinator between community and hospital programs.
- Utilizes crisis intervention/supportive counseling therapeutic techniques to help the patient/family cope with diagnosis, death and sexual dysfunction.
- Evaluates patient and family attitudes towards the pregnancy, diagnosis, complications, delivery and/or post-partum conditions.
- Participates in case management to assure continuity of care and services.
- Completes documentation of a psycho-social assessment as well as subsequent contact with the patient to streamline the coordination of patient care.
- Completes and maintains all necessary records pertinent to a case as part of the patient's clinical record.
- Acts as advocate for patients by assisting them in overcoming barriers to support services.
- Confers with all clinical members of the healthcare team in order to clarify the medical diagnosis and treatment plan.
- Determines adequacy of social services plan vis-a-vis the health care needed.
- Secures the interest and may coordinate the resources of community agencies whose aid is needed by patients or their family members.
- Initiates referral to appropriate support service (i.e.: social, health agencies) after assessing needs of patient.
- Must be able to cover various locations throughout the network based on business, practice and provider needs.
- Participates in and contributes to formal interdisciplinary rounds and conferences.
- Prepares appropriate statistics and reports pertaining to case load.
- Initiates referral to an appropriate support service (i.e.: social, health agencies) after assessing the needs of the patient.
- Teaches fellows, resident physicians, nurses and other staff the medical-social aspects of support services as related to gynecologic patients.
- Accesses protected health information (PHI) in accordance with departmental assignments and guidelines defining levels of access (i.e. incidental vs. extensive).
- Uses computer applications or other automated systems such as spreadsheets, word processing, calendar, e-mail and database software in performing work assignments.
Qualifications/Requirements:
Experience:
Minimum of 3 years (5+ preferred) where the primary function of the position was professional social work in a medical, mental health or human service setting as a member of a multidisciplinary treatment team, required. Experience in women's health, the department of obstetrics and gynecology, and/or with underserved communities is preferred.
Education:
Master's degree in Social Work (M.S.W.) from a School of Social Work accredited by the Council on Social Work Education, required.
Licenses / Certifications:
Possession of a valid license and current registration as an LMSW or LCSW, issued by the New York State Education Department, required.
Other:
Special Requirements: Must maintain a license and registration as an LMSW or LCSW throughout the course of employment in this title. Bilingual - Spanish preferred.
