Summary

The Advanced Practice Nurse-Cardiology Heart Failure located at University Drive serves all veterans with cardiac conditions from evaluating a potential diagnosis - to intervention - and post procedure management. Evaluation of individual patients is highly complex and involves the collection of essential clinical input from multiple services.

Duties

• The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
• The NP practices to the full extent of their education - training - and certification
• Core functions are outlined in the clinical privileges that correspond with this position
• The NP has prescriptive authority for a panel of patients or program as indicated by their position
• The Heart Failure Nurse Practitioner will cover all cardiology specialties to include - structural heart - cardiac Catheterization - nuclear medicine and arrhythmia clinics
• Major duties and responsibilities of the position but are not limited to: Independently manages patients with acute and chronic health conditions with an emphasis on cardiovascular health promotion and disease prevention using evidence- based practice
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with the interdisciplinary team
• Orders and interprets labs - tests - and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

• Completes E- Consults and coordinates with physicians Round with cardiac surgery on post TAVR patients - assist with discharge follow up/education Enter data in the VA VART-CL registry Participate in the management of discharge planning needs - including ancillary services Evaluate - development and implement treatment plans for inpatient /outpatient healty failure consults and provide continuity of care - including follow- up for patients seen by the cardiology fellow
• Complete History and Physicals ( H&P) Respond to consults - according to policy and procedure
• Determine the pathway to care including clinical evaluation and coordination of care
• Conducts telephone - face to face - VVC and CVT clinics
