Overview

• Facilitate patient involvement in treatment, education, and discharge planning through individual, group, and family counseling.
• Collaborate with healthcare team members to ensure safe, timely, and resourceful patient care.
• Assess psychosocial risks by evaluating prior functioning, support systems, reactions to illness, and coping abilities.
• Counsel patients and families on emotional, social, and financial impacts of illness/disability.
• Access and mobilize community and family resources to meet patient needs.
• Conduct mental health assessments upon physician referral and provide therapeutic interventions.
• Serve as the point person for cases involving child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence.
• Liaise with protective services, guardianship, foster care, and mental health agencies.
• Support complex discharge planning and crisis intervention, utilizing principles of crisis de-escalation.
• Develop and maintain relationships with community agencies, law enforcement, schools, and other stakeholders.

• Experience: Relevant health care, mental health, or community-based experience working with adult populations.
• Education: Master's Degree in Social Work, Counseling, Psychology, or related field required.
• Licensure: Current licensure or license eligibility in Indiana as an HSPP, LMFT, LCSW, or LMHC required. Clinical licensure preferred.
• Certifications: CPI (Non-Violent Crisis Intervention) certification may be required.
• Skills & Knowledge:
  • Knowledge of the disease impact, human behavior, and legal mandates regarding child and adult protective services.
  • Principles of growth and development across the lifespan.
  • Techniques for crisis intervention and de-escalation.
  • Strong communication, assessment, and counseling skills.
  • Ability to develop and maintain relationships with community organizations and agencies.
  • Proficiency in Microsoft Excel and Word.
