Overview

• Collaborate with medical teams to facilitate safe, effective patient care.
• Assess psychosocial, emotional, and financial needs of patients and families.
• Counsel patients and families on the emotional, social, and financial impacts of illness/disability.
• Access and mobilize community and family resources to meet patient needs.
• Conduct mental health assessments upon physician referral and provide therapeutic interventions.
• Support complex discharge planning and coordinate guardianship, foster care, and adoption placements.
• Serve as a liaison with child and adult protective services, law enforcement, and community agencies.
• Support patients through acute crises, utilizing de-escalation techniques to prevent violence.
• Provide crisis intervention in high-stress situations and support end-of-life, guardianship, foster care, and adoption needs.

• Education: Master's Degree in Social Work required.
• Licensure: Active LSW licensure required.
  • Temporary LSW will be considered.
  • Supervision for LCSW provided if applicable.
• Certifications: De-escalation certification required within 6 months of hire.
• Experience: 3-5 years preferred, but not required, in mental health, child and family services, adult protective services, Veteran's Administration, or substance abuse.
  • Knowledge of disease impact, human behavior, and crisis intervention techniques.
  • Familiarity with state and county legal mandates for protective services.
  • Principles of growth and development across the lifespan.
  • Proficiency in Microsoft Excel and Word.
  • Experience working with geographically and financially diverse patient populations.

• Days: Tuesday and Thursday or Wednesday and Thursday
• Weekends: Every other weekend (Saturday and Sunday)
• Hours per shift: 8:00 AM - 8:00 PM (12 hours)
• Total hours per pay period: 72 hours
