Overview

• Collaborate with medical and mental health teams to deliver safe, effective patient care and discharge planning.
• Assess patients' and families' psychosocial needs, including prior functioning, support systems, and reactions to illness.
• Counsel patients and families on emotional, social, and financial impacts of illness/disability.
• Access and mobilize community and family resources to meet patient needs.
• Conduct mental health assessments upon physician referral and provide therapeutic interventions.
• Support complex discharge planning and coordinate guardianship, foster care, adoption, and mental health placements.
• Serve as the primary contact for cases involving child abuse/neglect, adult/elder abuse, institutional abuse, and domestic violence.
• Liaise with protective services, law enforcement, schools, probation, and other agencies.
• Utilize crisis intervention, de-escalation, and interviewing techniques to support high-risk/high-stress situations.
• Develop and maintain strong working relationships with community partners and healthcare teams.

• Education: Master's Degree in Social Work required.
• Licensure: Permanent Indiana LSW licensure required; LCSW preferred.
  • Temporary LSW licensure considered, with full licensure expected within the first year of employment.
• Experience: 0-3 years of relevant healthcare, mental health, or community-based social work experience.
• Additional Skills & Knowledge:
  • Knowledge of the impact of disease processes and human behavior for assessments, crisis intervention, and counseling.
  • Understanding of state and county legal mandates related to protective services.
  • Principles of growth and development across the lifespan.
  • Proficiency in crisis de-escalation techniques.
  • Ability to develop strong relationships with community agencies, law enforcement, and healthcare teams.
  • Proficiency in Microsoft Excel and Word.
