About the opportunity:
Shaw Cancer Center (Dillon/Edwards): responsible for providing psychosocial oncology services to patients of both the Dillon and Edwards campuses.
Shaw at Home Program: acts as an active member of the palliative care team by providing psychosocial oncology services and interventions to support patients across the continuum of care.
Survivorship Program: acts as an active member of the Spirit of Survival team through program development and participation to enhance survivors? quality of life and to reduce risk of recurrence.
Clinical Role: This is a clinical social work position whereby the psychosocial oncology counselor/social worker allocates 100% of FTE time to clinical functions and direct patient care within the Shaw at Home program as well as the Shaw Cancer Center as an organization.
This position is Full Time working 32 hrs/week, Monday-Thursday 8am-4:30pm.
What you will do:
Utilizes evidence-based practice to function as a practitioner within the field of psychosocial oncology and palliative care. Provides comprehensive psychosocial assessments, palliative care consultations, and develops social work interventions and treatment plans for patients of the Shaw Cancer Center and the Shaw at Home program.
Provides support and counseling (1:1, couples, family, and group), assists patient and family with coping skills related to cancer diagnosis and treatment, recovery, palliation, end of life, and bereavement.
Responsible for contributing to the development of the Survivorship program. This is through participation in the development, planning, and implementation of survivorship services within the community to promote wellness and cancer risk-reduction initiatives and activities.
Responsible for care coordination with community referral partners
Responsible for addressing financial toxicity as it relates to cancer care. Does this collaboratively with financial navigator, as well as directly with patients through education and direct facilitation of Medicaid buy-in program application completion and serving as referral source for community non-profits.
Serves as liaison to staff to help problem-solve departmental issues/concerns to promote communication and cooperation.
Serves as a resource to colleagues of the Shaw Cancer Center for support and counseling to assist staff dealing with patient situations that can result in vicarious trauma or with the imbedded stressors of working within a healthcare environment.
Represents the Shaw Cancer Center and Vail Health by attending and speaking at community events, in marketing campaigns, professional meetings, and in various other media outlets.

What you will need:
Experience:
Two years of previous experience in psychosocial oncology required
License(s):
Required: State of Colorado Licensed Social Clinical Social Worker (LCSW) or State of Colorado Licensed Professional Counselor (LPC)
Certification(s):
Oncology Social Worker (OSW-C) preferred
Computer / Typing:
Must possess, or be able to obtain within 90 days, the computer skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc
Must have working knowledge of the English language, including reading, writing, and speaking English.
Education:
Required: Master?s degree in Social Work from an institution accredited by the Council of Social Work Education or Master?s degree in Counseling from an institution accredited by CACREP.
