Summary:
The Social Worker applies Generalist professional social work skills to the psychosocial assessments and interventions of individuals, families, and groups as appropriate to client needs. Works collaboratively as a member of the multidisciplinary team in addressing the needs of the patient/family, advocating for inclusion and equity.
Responsibilities:
Conducts psychosocial assessments of the patient/family situation.
Functions as a member of the multidisciplinary team and provides Generalist Medical Social work services to patients and their families to improve or maintain social, emotional function and physical health.
Provides case management for patients with emphasis on issues of Social Determinants of Health (SDOH) in collaboration with patient and caregivers, medical team, and community partners to provide the most appropriate and comprehensive care.
Consults and collaborates with community partners.
Seeks professional social work supervision when appropriate for practice or training.
Works in conjunction with Social Worker II, assisting with the duties and responsibilities performed by this position, with an emphasis on resource brokerage and referrals to community programs.
Responds to consults related to screening tools administered to patients.
Ethical and timely completion of documentation as expected in the context of the department.

Technical Expertise
Experience with and ability to navigate electronic medical records is preferred.
Experience working with all levels within an organization is required.
Experience in healthcare is preferred.
Proficiency in MS Office [Outlook, Excel, Word] or similar software is required.
Education and Experience
Education: Bachelor's degree in social work from accredited Social Work academic program is required.
Certification: Licensed Social Worker is required.
Years of relevant experience: 6-8 years is preferred. Pediatric experience is preferred.
Years of experience supervising: n/a
Experience working with culturally diverse individuals is preferred.
Experience with community resources used by families with children is preferred.
