Summary:

The Clinical Pharmacy Specialist provides specialized clinical activities related to Antimicrobial Stewardship (ASP) monitoring and oversight within the facility and works with the ASP committee on the local and system level. The pharmacist will focus on optimal selection, dosage, route, and duration of antimicrobial treatment. Some responsibilities may include, but not limited to review and monitor daily ASP alerts for de-escalation and bug-drug mismatch, document anti-infective related interventions, develop and maintain criteria for the appropriate use of antimicrobials, and review formulary requests for new antimicrobials. Additionally, this pharmacist may perform Medication Use Evaluations (MUE) related to antimicrobial use, provide staff presentations on ASP updates and best practices, and develop evidence-based guidelines, clinical pathways, and protocols to minimize the development of resistant organisms and to improve patient outcomes.

The pharmacist may also perform other types of medication utilization activities, including but not limited to anticoagulation, pharmacokinetics, dose adjustments, order verification, and IV to PO-conversions. Participates in committees and represents the department of pharmacy throughout the hospital settings within the area of focus.

Ideally, this pharmacist should be board certified, or residency trained with a background in ASP, or have clinical experience in ASP responsibilities
