The School Health Care Coordinator is responsible for coordinating and managing health services for students within the school system. This role involves identifying, assessing, and addressing health-related barriers to student success and well-being. The care coordinator works collaboratively with students, families, school staff, primary care providers, and school-based health provider to ensure that students receive the necessary health care services and bridge the gaps to health care.
Responsibilities:
1. Monitor and track student health progress, ensuring proper follow-up and support.
2. Provide health education and counseling to students and families to manage medical conditions and promote healthy behaviors.
3. Work closely with school nurses and school-based health providers to provide holistic support for students.
4. Collaborate with healthcare providers, mental health professionals, and community organizations to ensure students have access to needed services.
5. Coordinate referrals to medical, mental health, and social services for students and families.
Serve as a liaison between the school, students' families, and local healthcare providers to ensure students receive appropriate care and support.
7. Serve as an advocate for students and families to ensure they have access to necessary resources and services.
8. Assist families in navigating healthcare systems, including insurance, medical appointments, and follow-up care.
9. Provide crisis intervention and support in response to health emergencies, such as medical conditions, injuries, or mental health crises.
10. Maintain accurate and confidential records of student health information, services provided, and outcomes.
11. Prepare reports on health trends, issues, and outcomes for school administration and relevant stakeholders.
12. Comply with all regulations regarding student privacy, including HIPAA and FERPA.
13. All other duties as assigned.
Other information:
Technical Expertise:
1. Experience in pediatrics.
2. Proficient in MS office Suite (Outlook, Excel, Word) or similar software. Epic or similar EMR software is preferred.
Education and Experience:
1. Bachelor's degree in nursing (BSN) preferred. Associate's degree in nursing required with completion of bachelor's degree in nursing in 5 years from hire.
2. Current Registered Nurse (RN) license in the state of Ohio required; additional certifications in pediatric nursing or school nursing preferred.
3. Proven experience in pediatric healthcare, minimum of 2 years required. Preferred experience a school-based environment or community health setting.
4. Certification in Basic Life Support (BLS) CPR is required.
