Overview

• Collaborate with healthcare teams to ensure safe, timely patient care and discharge planning.
• Assess patients' and families' psychosocial needs, including prior functioning, support systems, reactions to illness, and coping skills.
• Counsel patients and families regarding emotional, social, and financial impacts of illness/disability.
• Access and mobilize community resources, including protective services, foster care, guardianship, and mental health placements.
• Conduct mental health assessments as ordered by physicians.
• Serve as the liaison between Indiana University Health and state/county child and adult protective teams.
• Serve as the point person in cases of abuse, neglect, domestic violence, and institutional abuse.
• Support crisis intervention and de-escalation to manage dangerous or high-stress situations.
• Assist with complex discharge planning and coordination of care.

• Education: Master's Degree in Social Work required.
• Licensure: Permanent LSW licensure required; LCSW preferred.
  • Temporary LSW licensure considered with full licensure expected within the first year.
• Certifications: De-escalation certification preferred.
• Experience: 0-3 years of medical social work experience required.
• Knowledge & Skills:
  • Knowledge of disease impact and human behavior for assessments, crisis intervention, and counseling.
  • Understanding of state and county legal mandates related to protective services.
  • Principles of growth and development across the lifespan.
  • Proficiency in Microsoft Excel and Word.
  • Relevant healthcare, mental health, or community-based experience preferred.
