Opportunities Available:

• Primary Care Providers (MD/DO, NP, or PA)
  • Deliver full-scope outpatient primary care services
  • Practice in a multi-disciplinary care team (behavioral health, dental, medical, and care coordination)
  • Manage both acute and chronic conditions, with an emphasis on health equity and prevention
  • Use Advanced Access scheduling and shared decision-making tools

• Urgent Care Providers (NP or PA)
  • Provide episodic care for non-emergent conditions in our same-day care settings
  • Treat minor injuries, infections, and urgent complaints within an integrated system
  • Collaborate with on-site primary care and behavioral health providers for seamless continuity of care
  • Ideal for providers who enjoy fast-paced clinical work in a structured, team-oriented setting

Who We’re Looking For:

• Physicians (MD/DO) – Family or Internal Medicine trained; Nurse Practitioners (APRN) – Family Nurse Practitioner certified; Physician Assistants (PA) – Graduate of accredited PA program
• Licensure or eligibility in Connecticut
• Board certification (or eligibility with plans for timely completion)
• Commitment to serving underserved populations
• Bilingual English/Spanish skills strongly preferred
